What are you guys using to debug permissions?

I run quite a few plugins on my server, often I'm confused as to what is blocking a player access to something. And unfortunately, some plugins have undocumented permissions, it's rare, but I've run into it before.

Does anyone know of a plugin or method that can debug or output to the console or chat when a player is missing a permission, and display the permission they require?
