Permissionsex not giving me prefix

On my server i set my self to a sertain rank to see if all of the permissions work and for some reason on my first rank i can type in chat but it comes up like this: "<XXXXXXXX> hi" and no prefix. on my second rank i cant type in chat at all. Any thoughts on how to fix this?

now its happening to all of my ranks besides Owner. i need this to be fixed fast
