Minecraft Server Keeps crashing after 5 minutes

My Mc server Is just a survival server that just keeps on crashing. I have been using this for a month now and i didn't change anything but for some reason it just keeps crashing after awhile. Or if 2 or 3 people are playing it just randomly "times out" And nobody can join back. Then it crashes. Please help me out this is my One and Favorite server. everytime it crashes Just before it does nobody have use commands or open chests,signs,exc

Essentials doesn't work properly on 1.12. Remove all Essentials plugins and use EssentialsX instead.

Also the version of DisguiseCraft you have installed isn't compatible with 1.12.

Essentials does not support 1.12, has not been updated in years, and is crashing your server. Use EssentialsX. instead.
