    How do I do this that is in this img? I really need help.. Like what is the plugin to do this?

    Symbiosis

    I don't think you need a plugin. If you place a spawner egg into an anvil and rename it then use the spawner egg, it will spawn mobs with the new name given. If it does not work with withers, there is probably a mob renamer plugin or something of the such out there.
