Custom potions

Im wondering if theres a way to make custom potion effects, I know how to do PotionMeta and stuff but how can I make it different in the GUI make this: http://prntscr.com/7bqvra Have a custom name. If you have any ideas ill appreciate all the help I can get. Thanks!

I figured theres a way to change it with packets?

There's only so much you can do with packets.
In vanilla Minecraft, there's a class called Potion (net.minecraft.potion.Potion), that stores all possible potion effects. So unless you're up for a mod-plugin combo, you are stuck with what exists. You can, however, use a resource pack to change both the icon and title of an existing potion effect!
