Okay, I will try to do it. Sorry for the first plugin.

It will change the item in hand, I dont have to replace it....

Why would you want to do it in general? The request has already been filled.

Why would you want to replace it or change the Item in a Players hand at all...?
We are talking about "adding modified items to the Players Inventory when doing /i".
Not about "Changing the lore of a random item while completely ignoring the functionality and the request of the OP".
And as you already heard, this isnt easy but there is already a Plugin which allows you to do that, so why do you still spend time to try filling a filled request?
