Create Custom Item based on existing item?

Hello there,
I did look through the forums but couldn't find anything that matched what I'm after... I'm fairly well versed in Java, but not very well versed in Bukkit (I've made one mod and that was the tutorial one), and I'm looking to know if it's possible to make a custom item...
It would use an existing items texture, but have a different name and a custom behavior, for example look like a stick but called "rod of doom" and restore all your health when you "use" it (without commands). Is this even possible without spout?
It's easy to do with a normal Minecraft client mod, but servers are a whole new realm of tricky...
