Have you set up a port-forward in your router?
    Has your ip changed?
What about you internal ip on the home network(they happens to change a lot :S ) has it changed?
Is port 25565 open in your firewall, both router and computer?

How do you set up a port-forward?

That's actually not that hard, though many people think it is ;)
You just go to http://portforward.com/ and find your router on the list.
After this you will get an advertisement, just skip it with the link in the upper right corner.
Then you will get a list of programs/games to portforward for, choose minecraft server, and there you go, a step by step guide on how to portforward with your router :)

Maybe your internet provider is blocking the port, try and contact them to see if this is the case :)

This is most likely due to not having a static IP on your LAN. I would suggest following a guide to set one up, if you're going to be running a server. The first one to pop up is http://portforward.com/networking/static-win7.htm
Follow it if you like. I highly recommend it. Just make sure you follow every step very carefully, as you don't want to mess anything up.
